RIRS :

Retrograde Intrarenal Surgery
RIRS is an endoscopic non invasive procedure done by flexible fiberoptic endoscope.In RIRS the scope is placed through the urethral opening into the bladder and then through the ureter into the pelvi-calyceal system the kidney. The scope can be directed to any calyx like upper, middle or lower calyx for diagnosis as well as therapeutic procedure.

Procedure :

It is done under spinal or general anaesthesia

With help of cystoscpy, Guide wire passed in to ureter , followed by ureteral dilatation done with balloon dilater. Flexible Teflon access sheath passed into ipsilateral ureter on guide wire with help of C-Arm Machine.

Through which flexible ureteroscpe is passed up to pelvi-calicealsystem

Ans diagnostic as well as therapeutic procedure.

Uses:

Dignostic procedure:-
1)Malignancy

2) Stone disease

3) Obstruction:- Caliceal stenosis, PUJ or VUJ narrowing

Therapeutic procedure:-
1) Stone removal

2) Malignant mass fulguration

3) Residual fragments removal

4) Stricture dilatation

Advantage :

  2. Faster recovery
  3. No blood loss
  4. Less painful

Disadvantage :

  1. Costly
  2. Contraindicated in infection

RIRS is performed by a specialist, an uro-surgeon with special expertise in RIRS. The procedure is usually done under general or spinal anaesthesia.

The advantages of RIRS over open surgery include a quicker solution of the problem, the elimination of prolonged pain after surgery, and much faster recovery. No Blood loss.
